Thanks for your interest in this project, which offers free help for people in Manitoba who have gambling problems. This project has clearance from the University of Windsor Research Ethics Board and adheres strictly to research ethical principles. 

Please note that you can either fill out this survey or we can go over it with you by phone if you prefer. You can also email us (info@seekingsafety.org) or call us (617-299-1610). Please be sure to provide a phone or email-- and know that everything you write here is entirely confidential and there are no right or wrong answers to this survey. 

Step 1: Fill out this brief questionnaire.  It does not obligate you in any way-- it is just the first step. You can also read, if you want, a detailed document that describes what the study involves and also what you would receive. 

Step 2: We will get back to you with further information. You can then decide if you want to participate.

Step 3: You would complete a set of measures online and would be paid $40 CD (as a gift card to amazon.ca) for completing them, and then would move on to the rest of the project.
