This is a voluntary questionnaire from the MAD-ID research network designed to update our member database and is not a part of any active research study. It should only take about 10 minutes to complete.
The mission of the MAD-ID Research Network is to promote the understanding of and advance clinical practice that optimizes prevention and treatment of infectious diseases through individual and collaborative research efforts of the Network membership. To-date, a number of research initiatives have been undertaken and completed through participation of Network members. Research ideas from members are welcome and will be fully considered by the Network leadership.
The information provided will be used to update our membership list and to identify and contact members for future research opportunities with MAD-ID. None of this data will be shared with any outside entity, but a summary will be shared with the MAD-ID membership. If you have previously completed the MAD-ID sign-up survey in 2012, please complete this new sign-up/survey to update your information with us. In doing so, you are informing us of you potential willingness to take part in MAD-ID Research Network study opportunities. This is no way obligates you to participate in these studies but you will be presented with the option.
