Please use this form to submit questions for the Opioid Forum on Saturday, August 19, 2017 from 10:00 am – 11:15 am. Clay Stamp, Executive Director of the Opioid Operational Command Center, will be answering questions submitted through this survey and from the audience about the heroin and opioid crisis that is gripping our communities. Priority will be given to pre-submitted questions.
