Introduction

We would like to thank all those who completed our Synagogue Reopening Survey. The results of the survey indicate that there are many of our congregants who wish to attend in-person High Holy Days Services.
Due to the requirement for Physical Distancing and to be in compliance with all COVID-19 related regulations, we are limited in our ability to accommodate all of our members who may wish to attend in-person High Holy Days services.
We ask you, in this short questionnaire to rank the services you wish to attend. We will try our best, but cannot guarantee that you will be accepted to your service selections.
For those Families with Children, please complete the section for Families with Children Programming
All those accepted will still be required to complete and pass the Synagogue COVID registration process no more than 72 hours before the High Holy Days and pass a second screening at the door at the time of entry. Everyone entering the synagogue premises is required to properly wear a mask at all times.
Those not accepted for their selected service will be able to attend and participate via the live-stream of our synagogue services.
If you are accepted to attend specific services or programs, you will be notified in early September and given further instructions.
If you do not complete this form, we will assume that you will be attending High Holy Days services using the Synagogue Live-Stream only.
We look forward to the time when we can welcome back everyone to our synagogue.
