2017 CHIA Nomination Form

New and on-going partnerships are vital to assure conditions exist where people can be healthy. We constantly hear of individuals, groups, and organizations who are working to improve the health of Clay County, and thus the Community Health in Action Award is meant to honor those working to improve overall community health and create positive outcomes. This year we will be accepting nominations for 6 categories.

All nominations should be submitted no later that Thursday August 31, 2017.
