Math and Science Attitude Survey

The first four questions of this survey were selected from an adaptation of the “Science Motivation Questionnaire II © 2011 Shawn M. Glynn”. It has been adapted to include both math and science classes. Questions 8 and 9 are a selection of items from the "Revised Math Attitude Survey by Reid, P.T., and Roberts, S.K. (2005), which were adapted to include both math and science.

In many of the questions below, there are a series of statements. Select the answer that shows how much you agree with the statement. There are no “wrong” or “right” answers. The only correct responses are those that are true for you. Whenever possible, let the things that have happened to you help you make a choice. Do not spend too much time on any one statement.
