Affiliate Questionnaire

Dear Affiliate OAO Member,

We are reaching out to each of you to complete a brief survey about how you participate as an Affiliate member of OAO.  We are looking to improve and possibly change various aspects of your membership, so your input is very important.  This survey is anonymous, so please take a few minutes to help us out.
1.How long have you been an OAO member?
2.What are your expectations of your relationship with OAO? Or why did you become a member of OAO?
3.What do you like most about OAO?
4.Have you ever attended an OAO event as a vendor?  If so, which ones?
5.Do you like the theme aspect of the OAO events?
6.Which days do you attend an OAO event?
7.Do you feel the OAO members interact with you at Events? (stop by your table, etc.)
8.What recommendation would you make to get more interaction from OAO members at events?
9.Would you consider getting involved with OAO? (i.e. volunteering on a committee, speaking at an event, etc) If yes--we will collect your name, phone, and email to contact you.
