Purpose of Study

This study serves as the first component of the Principal Investigator's (PI) dissertation research.  The purpose of this component is threefold: 1) to develop an understanding of the most common referrals for psychological and neuropsychological assessment in the PAST YEAR; 2) determine differences between suicidality among these referrals; and, 3) to understand practicing clinical psychologists' and neuropsychologists' views of their roles in suicide risk assessment, including knowledge of neuropsychological correlates and how these may differ from depression or suicide ideation (SI) alone, perceived utility of neuropsychological test results in assessment of suicide risk, and interest in suicide risk assessment as a primary referral question.

There are two versions of this questionnaire with only minor variations between the one designed for Clinical Psychologists and the one for Clinical Neuropsychologists - referral questions inevitably differ and, therefore, questions in this realm were tailored.  With this being said, there are three (3) criteria that must be met in order to be eligible for this questionnaire:

1) You must have earned your doctoral degree from an APA- or CPA-accredited university;
2) You must be a Clinical Psychologist licensed in your state/province; and,
3) You must have at least one year of practicing independently.

This 57-item questionnaire (which includes informed consent, inclusion criteria, and verification of submission) may take approximately 20-25 minutes to complete.  You will first review information related to informed consent of your voluntary participation in this study and the measures taken to ensure confidentiality and privacy.  You will then be informed of the questions to be asked of you.

Your time and participation are greatly appreciated - your responses are invaluable.  Should you have any questions related to the study questionnaire or results, please do not hesitate to contact the PI or dissertation Chair via emails listed below.
