In 2015, the Canadian Mental Health Association Ontario in partnership with the Wellesley Institute, John Howard Society of Ontario, and with support from EENet at Centre for Addiction and Mental Health formed a Community of Interest (COI) to advance the agenda on the issue of providing housing support to individuals with mental health and addictions issues involved with the justice system.

More specifically, the Housing, Health and Justice COI focuses on individuals with diagnosed or undiagnosed mental health and addictions issues who have been in correctional custody for any length of time, and as a result, face barriers in accessing or retaining affordable or supportive housing. As part of the work of the Housing, Health and Justice COI, we are currently conducting a scan of housing programs across Ontario that are serving this specific population.

Please take a few minutes and fill out this brief survey if your organization provides housing support to individuals who have spent any length of time in correctional custody and have diagnosed or undiagnosed mental health or addictions issues. If your organization does not run a housing program for this population but you are aware of other housing programs that serve this population, you can still fill out this survey.  We are looking for a general profile of these housing programs, for example, the goals, activities and outcomes of the program.

If you would like to share the profile of more than one housing program, please submit the first profile, and then use the survey link that was provided to you to share the profile of the second housing program. You can use the same survey link and take the survey again to submit the profiles of multiple housing programs.
