Dear Residents of the Lyngblomsten Care Center,
Lyngblomsten is seeking feedback on your experience as a resident of the Lyngblomsten Care Center. Your responses will help us identify what we're doing well and where we can improve, all with the goal of providing you with the best person-centered experience possible!
Please answer the questions that follow, reflecting on your experience as a resident of the Lyngblomsten Care Center. If you receive services from another Lyngblomsten entity (e.g., your spouse lives at The Heritage at Lyngblomsten, you participate in a caregiver support group provided by Lyngblomsten Community Services), you will receive a separate survey link for that entity. Please use this survey to reflect on your experience as a resident of the Lyngblomsten Care Center.
The survey is short (it takes about 5 minutes to complete) and will remain open from Wednesday, May 20, through Tuesday, June 2.
If you need assistance completing the survey, you may ask a family member or friend to help. However, we ask that only one survey be submitted for you (your family member/friend should not submit their own survey in addition to yours).
Your input on the survey will be confidential/anonymous. After completing it, if there is any topic you would prefer to discuss in greater detail, please contact Trisha Gerleman, Administrator of the Lyngblomsten Care Center, at (651) 632-5302 or tgerleman@lyngblomsten.org. She would be more than happy to speak with you.
