MO HCBS Children with DD Waiver Survey 2016 (Survey Closes April 1, 2016 at 9:00 PM)

The Center for Medicare and Medicaid Services, known as CMS, has made changes to its requirements for home and community based services. The new final rule, effective March 17, 2014, requires states to evaluate its HCBS settings to meet the new rule’s definition. The new Final Rule affects all HCBS settings (residential and nonresidential) that are controlled, owned and operated by providers in which individuals receive home and community based services.

To ensure compliance with the new rules, the Department of Health and Senior Services, Department of Mental Health, and Department of Social Services is requesting all providers of HCBS services complete the survey. The survey results will be assessed and the information gathered through this survey will be used to develop and update the Transition Plan. Please answer the questions with the type of activities that are “typical” of the setting type. Comments can be added to the survey if you would like to provide additional information.
