1. Introduction

Thank you for participating in this survey on mental health and substance abuse within the Coachella Valley.

This survey is being conducted by HARC (Health Assessment and Research for Communities) on behalf of the Betty Ford Center. Both organizations are nonprofits located in the Coachella Valley.

We are conducting this survey of local community leaders so that we can better understand the health needs and barriers to care for those with mental health and substance use conditions. You have been invited to participate because you are a community leader that represents and/or serves a substantial portion of our local population. Even if your work is not strictly in the mental health/substance abuse treatment field, we still want to hear from you to learn from your expertise working with people in the Coachella Valley. 

The survey covers areas such as demographics, perceived problems, consequences, and barriers associated with mental health and substance use in the Coachella Valley.

You can skip any questions that make you uncomfortable, and you can quit the survey at any time. The survey will take approximately 10 minutes to complete.

Your responses to this survey will remain confidential. This means your words will never be connected to your name, organization, or other identifying information. Trained researchers will handle your data with care to make sure all of your information is protected.

At the end of the survey you will have the opportunity to enter a drawing for a $100 Visa gift card.
