COVID-19 Vaccine Signup - Laurel Health Centers

The Laurel Health Centers have COVID-19 vaccines available at all center sites, including pediatric vaccine and booster shots. Anyone aged six months and up can receive a COVID-19 vaccine. The vaccine is safe, effective, and no cost to you. To register, please fill out the short survey below and our staff will be in touch ASAP. Note: You do not have to be a Laurel Health patient to schedule your COVID-19 vaccine with us. Our medical professionals strongly encourage everyone eligible to receive their vaccine as soon as possible to protect themselves and others. Most patients who are hospitalized with COVID-19 are unvaccinated. Being vaccinated greatly reduces your risk of serious COVID-19 symptoms, hospitalization, long-term chronic complications, and death.
