Please fill out this application in its entirety and attach the required documents. After submitting this application you will be contacted by the Lamaze office and asked to pay for your application. Upon payment, your application will be sent to subject matter experts for review. 
Please note that submitting and paying for an application does not guarantee that your application will be approved. Applications are evaluated with the same standards by which Lamaze Continuing Education offerings adhere to. Please apply at least 60 days prior to your event.
