Questionnaire to the population in the scope of the project "Municipal Health Plan and the Development Plan for the Health and Well-being Cluster of the municipality of Lagos"

As part of the project "Municipal Health Plan and the Development Plan for the Health and Well-being Cluster of the municipality of Lagos", which aims to characterize the health profile, social determinants of health, access to and use of health services, the National School of Public Health of NOVA University of Lisbon (NSPH-NOVA) in partnership with Lagos Municipality intends - through this questionnaire - to survey the opinion of citizens living in this municipality about their experiences and perceptions associated with health and quality of life.

This questionnaire is anonymous and NSPH-NOVA is sole the institution responsible for data collection and analysis. Data analysis is intended solely to pursue the aforementioned objectives and confidentiality is guaranteed.
