We welcome your views

We would like to invite you to complete this survey, looking at the care we provided to your loved one while they were with us in the last days of life at The Queen Elizabeth Hospital. We know this is a difficult time for you but your experiences are important and will help us improve future care for patients and families. This questionnaire is not intended to cause you any distress and we very much hope that you feel you will be able to respond.

These questions help us to understand more about the care your relative/friend received in the last days or hours of their life.
