Evaluating the Effectiveness of the Pro Patria Centre Model
By completing this form I give consent to participate in this research project.
In giving my consent, I confirm:
1. The project and time commitments have been explained, and my questions answered.
2. I have read and understood the Information Statement.
3. I know I can withdraw at any time without affecting my relationship with the organisation or researchers.
4. I understand my participation is confidential, and my identity will not be revealed.
5. Participation is voluntary, and I’m not obligated to consent.
6. My de-identified data may be shared or used for future research.
7. I understand this study is being conducted as part of the Commonwealth Government’s funding conditions to measure the impact of these programs.
8. I understand that the data collected will be used to advocate for veterans, first responders, and their families.
9. I understand that the data will provide critical information to supporters, grant providers, and other stakeholders about the positive impact of their financial and in-kind contributions.
10. I understand that de-identified data will be shared with medical and allied health providers to inform them of participant progress and to optimise the client journey through Pro Patria Centre programs and services.
11. I understand that the Resilience Report is primarily for my personal use, helping me understand and track my wellbeing and resilience over time.
12. I understand my participation is guided by relevant Pro Patria Centre Ltd policies, including the Privacy Policy, Confidentiality Policy, Records Management Policy, and Client Journey with External Providers Policy and Procedure.
