If you wish to remain anonymous, you do not have to provide your name or any personal information in this questionnaire. Any personal information you choose to provide will be considered confidential (your personal information is collected consistent with section 26 (c) of the Freedom of Information and Protection Act).
Completed questionnaires are kept in a secure location, are never placed on personnel files and will not be shared with your principal/supervisor’/manager without your authorization. However, please note that issues identified that may pose a serious or immediate threat to persons, public safety, or the environment will be reported as required in order to deal appropriately with the issue.
Statistical summary reports consolidating information from all exit interviews will be completed annually to identify general trends and issues. This information cannot be used to identify individuals.
If you prefer, a copy of your exit interview can be shared with your principal, supervisor or manager. If you would like to have a copy of the exit interview shared with your principals, supervisor or manager, please indicate below. Otherwise, you do not have to provide your name.
