SAMPLE: The Family Exit Survey is designed for parents whose child is leaving the school. It includes FOUR Sections.

Section ONE: About the school - rating scale for 35 questions
Section TWO: Written responses and some demographics - 7 questions
Section THREE: Reasons for leaving - rating scale for 16 questions
Section FOUR: General comments - space for writing any further feedback
