1. RESEARCH PARTICIPANT CONSENT FORM

PILOTS’ WILLINGNESS TO PILOT A MANNED AIRCRAFT IN UNMANNED AIRCRAFT SYSTEM INTEGRATED AIRSPACE AND AIRPORTS

Purpose of this Research: I am asking you to take part in a research project for the purpose of collecting attitudinal data on pilot perceptions and willingness to pilot an aircraft in unmanned aircraft system integrated airspace and airports. During this study, you will be asked to complete a brief online questionnaire about your perceptions and willingness to pilot an aircraft during a series of hypothetical scenarios. The completion of the survey will take approximately less than fifteen minutes.

Eligibility: To be in this study, you must be at least 18 years of age, and a pilot of any rating and experience level and must have piloted an aircraft within the last six months.
 
Risks or discomforts: The risks of participating in this study are no greater than what is experienced in daily life.

Benefits: While there is a possibility to participate and win in the drawing for a drone, other than that, the study will not benefit the participants personally. This study is intended to benefit academicians, regulators and technology developers, specifically contributing to safe integration of unmanned aircraft systems into the national airspace.

Confidentiality of records: Your individual information will be protected in all data resulting from this study. Your responses to this survey will be anonymous. No personal information will be collected other than basic demographic descriptors. The online survey system will not save IP address or any other identifying information. In order to protect the anonymity of your responses, I will keep your responses in a password-protected file on a password-protected computer. No one other than the researcher will have access to any of the responses. Information collected as part of this research will not be used or distributed for future research studies. Any personal information provided by participants interested in optional prize drawing will be securely disposed once the winner has been selected.

Compensation: There is no compensation offered for taking part in this study. There is an optional drawing to win a DJI Quadcopter Tello drone. Any information collected will be ONLY used to select the winner and once the drawing has been completed, all identifying information will be securely disposed. Names and any identifying demographics will not be released to the public or be able to be matched to any data after the study and drawing for a free drone is complete.

Voluntary Participation: Your participation in this study is completely voluntary. You may discontinue your participation at any time without penalty or loss of benefits to which you are otherwise entitled. Should you wish to discontinue the research at any time, no information collected will be used.
