Cancer Consortium Demographic Survey

As part of NCI’s new requirement for all of the nation’s NCI-designated Comprehensive Cancer Centers to include a Plan to Enhance Diversity in their Cancer Center Support Grants (CCSG), it is essential that we collect detailed demographic data from all members of our Fred Hutch/University of Washington/Seattle Children’s Cancer Consortium.

This is time sensitive as we are currently in the process of preparing the competitive renewal for our CCSG. So we are asking all members to complete this survey, though responses to each demographic question are optional. If you have any questions or concerns related to this survey please reach out to ofad@fredhutch.org.

This survey will take no more than 5 minutes to complete. Data will be reported to others only in aggregate form such that no individual can be identified. Individual identifying data will only be visible to the Associate Director for Diversity, Equity and Inclusion (Dr. Christopher Li) the Program Manager overseeing this effort (Ms. Lauren Fielder) and Cancer Consortium admin.
