Meals Survey

The purpose of the Susan Mast ALS Foundation’s Meals Program is to provide the primary caregiver with a break from cooking.  If you completed the survey in 2022 but your needs have changed, please complete a new survey. In 2023, we plan to prepare and deliver care baskets full of freezer meals and other goodies several times during the year. 

It would be greatly appreciated if you could please take this short survey to let us know a bit about your meal preferences.
