Welcome to the Mencap Liverpool '(Did they) Make it Better' Survey
Welcome to our short survey about the hospital experience of people with a learning disability.

WHY ARE WE DOING THIS SURVEY?
Last year Mencap Liverpool did some research and published a report about the hospital experiences of 12 people with a learning disability who had been to a local hospital between March 2007 and March 2010.
The report, called 'Make it Better' was launched in June last year. Over 80 people from the local hospitals came to listen to our recommendations. The recommendations were all made by the people we interviewed as part of the research and many of them were similar to those in the national Mencap campaign called Getting it Right.
Now we want to know whether things in hospital are better, for people with a learning disability and their family carers.
WHAT WILL WE DO WITH THE INFORMATION YOU TELL US?
The results of this questionnaire will be presented to the hospitals and the Primary Care Trust at a meeting at the start of October. They will be published on our website and sent to you if you give us your name and address on the last page.
The results may also be included in the annual Health Self Assessment Framework for people with learning disabilities.
You do not have to give your name and contact details, but if you do, we promise they will not be made public or given to anyone outside of Mencap Liverpool.
ABOUT THE QUESTIONNAIRE
There are only seven questions and it should take you around 5-10 minutes to complete.
The questions relate to the recommendations in the Make it Better report. If you want to give examples or tell us something else about your hospital experience please use the space provided in question 6.
This survey will be open until midday on Friday 16th September.
If you are a carer/support worker, please respond with or on behalf of the person with learning disability.
