The JCPH Travel Process Quality Improvement team is requesting that you complete the survey below. This survey will ask you questions about your experiences with the travel process at Jefferson County Public Health. Your answers to this survey will remain anonymous. Travel, for the purpose of this survey, is defined as any work-related travel requiring an overnight stay, and does not include local travel (i.e. meetings/conferences in Denver).
