We are pleased to support family councils and family member meetings. Please complete the below brief form so that we can set up a Zoom meeting for you. Family members can attend these meetings via phone or video conference on their computer, phone, or tablet.

* 9. LTCCC considers family council meetings private. Please indicate below if you are comfortable with a staff member joining the meeting.  If you say yes, we will let you know if we are able to attend.

You will receive an email with the Zoom meeting link and call in information at least three days prior to the meeting.  If you don't, please email info@ltccc.org.  

We would appreciate you telling other family council members about us and encouraging them to sign up for our news and alerts, www.nursinghome411.org/join.
