(Required.)
3.Your details
4.Nominated innovation and recipient
5.Tell us about the innovation and its context

Briefly describe the setting in which the innovation was developed and implemented, the people it was intended to benefit, the scale and stage of implementation, and any relevant opportunities, constraints or resources.

Your answer Maximum 250 words or approximately 2 minutes of audio/video.
6.What is the innovation, and what makes it genuinely different or significantly improved?

Describe the innovation and the strongest evidence that it represents a meaningful change in its context rather than an idea alone.

Explain:

• the problem or opportunity and the information showing that it needed to be addressed;

• the approach taken and how it differed from or significantly improved previous practice;

• how patients, learners, users, staff or partners were meaningfully involved;

• how the innovation was implemented in practice; and

• how evaluation and learning were used to refine it.

Use specific examples and relevant dates. Identify the supporting document or documents in section 9 that substantiate the need, the implemented approach, involvement of users or partners, and any evaluation or refinement.

Your answer Maximum 600 words or approximately 5 minutes of audio/video.
7.What difference has the innovation made, and how do you know?

Explain the documented outcomes and learning from the innovation, why they matter and who benefited.

Include:

• the difference made to care, services, education, training, access, workforce or professional development;

• who benefited and why the outcomes were significant in context;

• how you know - for example, before-and-after information, evaluation, audit, outcome data, adoption records or relevant feedback;

• what has been learned, including any limitations or changes made; and

• what is known about sustainability, adoption or application elsewhere.

Give the relevant findings and identify the supporting document or documents in section 9 where the outcomes, learning and any adoption can be verified. Potential benefit alone is not sufficient.

Your answer Maximum 600 words or approximately 5 minutes of audio/video.
8.How has the innovation strengthened practice or the profession beyond its immediate outcomes?

Describe the wider value created by the innovation beyond its direct users or original setting.

Relevant examples may include:

• workforce or capacity development;

• training or mentoring;

• improved access or a new model of care;

• sharing learning or enabling adoption elsewhere; or

• another contribution strengthening practice or the profession.

Give specific examples, explain the wider value created and identify the supporting document or documents in section 9 that substantiate the contribution and its effect.
