SAMPLE: The Transition Primary to Secondary School Survey is designed for parents whose child is transitioning between Primary and Secondary or Middle School. It is relevant whether the child is staying at the school, or leaving to go to another school. It includes FOUR Sections.

Section ONE: About the school - rating scale for 35 questions
Section TWO: Written responses and some demographics - 6 questions
Section THREE: Reasons for leaving - 15 tick box questions with room for comments
Section FOUR: Reasons for continuing at the school - 10 tick box questions with room for comments
