The Lakeshore Regional Entity is evaluating the potential of using validated and reliable assessments to aid in determining the appropriate level of support for an individual with intellectual and/or developmental disabilities. These assessments would address areas such as home living, community living, lifelong learning, employment, health and safety, social activities, and protection and advocacy. The assessments would also consider both extraordinary behavioral and medical health issues when determining needs.   

The reason for the evaluation is the idea that, determining an individual’s needs and strengths will improve the alignment of appropriate services and supports, resulting in effective care.  

In this survey, we ask you questions related to this to gain your perspective as a guardian or family member of an adult with intellectual and/or developmental disabilities. Your responses will be anonymous and used to help us plan for future services in the communities we serve. We also intend to gather consumer perceptions on this topic directly from consumers. This short survey should take less than 10 minutes to complete.
