About the award program
Credentialled Diabetes Educators (CDEs) are Australia's 'go to' qualified healthcare specialists for people with diabetes who can provide a person-centred care approach to diabetes education and care. They work with people with diabetes, empowering and assisting them in dealing with daily self-management. Recognition as a CDE is an assurance to you that these healthcare practitioners will provide you with high-quality diabetes education and expert advice to help you stay well every day.
The CDE of the Year award program is designed to recognise and honour achievement and contribution of CDEs in the provision of high-quality diabetes education and expert support for people living with diabetes. This award program is run by the Australian Diabetes Educators Association and financially supported by Eli Lilly.
If you think that your CDE is outstanding and makes a strong contribution to diabetes education, nominate them for this prestigious award.
Some examples of an outstanding CDE is someone who has:
- Helped you (or the person with diabetes who you care for) to develop an effective diabetes self-management plan
- Empowered you (or the person with diabetes who you care for) to be confident in managing diabetes
- Provided you (or the person with diabetes who you care for) with ongoing support to sustain the diabetes self-management plan
- Helped you (or the person with diabetes who you care for) with access to healthcare services to improve health outcomes
This nomination form is designed for people with diabetes, carers and family and it takes approximately 15 minutes to complete.

The information you provide will help us to select the winners and will only be accessible for Members of the Judging Panel and relevant ADEA staff. Your responses will be held in strict confidence and will not be used for commercial purposes.
