To broaden the thinking of our consumer representatives on the CCPC you are asked a 2 minute survey too:

1.       Make improvements.
2.       Broaden the influence of the public in WCHN decision making.

You will also be invited to register for the e-newsletter, which is a monthly memo that is sent out by the Consumer and Community Engagement.  Please see below for some common questions and answers you may have before completing the survey.

The survey is voluntary

If you would like to provide additional feedback, be involved in and or to make a formal complaint please visit the Consumer Feedback area on the website.  

When you submit your responses it will automatically be seen by the Director of Consumer and Community Engagement. The information will be used as part of a report that is written for the Consumer and Community Advisory Group. This group meets six times a year. 

This survey is monitored frequently but if by completing the survey it has raised any thoughts you are encouraged to speak to your healthcare provider.

Please note that due to the nature of the Internet, it is possible that data transmitted online can sometimes be accessed by unauthorised third parties as with any online information.  While every effort is made to minimise this risk, we cannot guarantee that the information you provide online will never be compromised.  Once the data is retrieved from the survey software provider it will be securely stored on password protected computers and password protected electronic files which will be accessed by the Manager of Consumer and Community Engagement. This survey is using SurveyMonkey to collect the data. Data collected via SurveyMonkey is held in the United States.  You can access SurveyMonkey’s terms of use here. Women’s and Children’s Health Network will  hold the data for a 12 month period.
