Thank you for your recent L&OD request for organizational development/consultation services. 

As a first step in the process, please fill out the brief survey below with as much information you currently have available. Once this information is received, we will reach out to schedule an initial intake/consultation meeting with you and/or your staff you identify below. 

Please note: Any information provided in this form will be kept strictly confidential and will only be used for purposes of preparing for the upcoming meeting with L&OD staff. 

After submittal, please allow up to 48 business hours for a response from L&OD staff.
