I, Colin Longworth (as the psychologist and group facilitator) want to get some ideas about what participants would want to look at and within those options what are more or less important overall. (Which includes an "other" option for a subject you might want to have added, that's not there already.) As well as get some more general enrolment information.

* 4. Is the time (6pm>7.45pm~), frequency (fortnightly, on a Thursday night), cost ($75 total with a $33.60 Medicare rebate, resulting in a $41.40 "Gap") and proposed location (Canning Vale) something that will work for you?

To enable Medicare to pay rebates, your GP referral, including your Medicare Card number and expiry date will need to be supplied separately before the first meeting and can be faxed or emailed.

Payment will be needed beforehand either on the night via credit/debit card, or by direct credit to my specified bank account. Medicare processing would normally occur within 24 hours of the end of the group.
