You are invited to participate in a research project entitled: "10 Years after Cameron: Have we made gains in improving the disclosure of harmful patient safety incidents in Newfoundland and Labrador?"

The purpose of this project is to explore the disclosure process from the perspectives of the multiple players involved and the potential role of trainees (students and residents) in the institutional disclosure process at Newfoundland and Labrador hospitals.

The Research Team consists of: Dr. Julia Trahey, Dr. Heidi Coombs-Thorne, and Dr. Vernon Curran.

You are asked to complete a survey, which will take approximately 10-15 minutes. Participation in this project is voluntary and you may withdraw from participating at any time. Please be assured that completion of the survey is anonymous and there are no identified risks to completion.

Information collected for this study will be kept for five years on a password-protected computer at the Medical Education Scholarship Centre, Memorial University.
