This survey is intended for our partners to share pandemic related changes and challenges you are seeing in older adults and adults with disabilities. We have another survey specifically for older adults – please contact us if you would like a copy or link to send out. You can also respond to this survey by telephone: call 1-866-720-4863 during business hours and a staff member will happily take your responses over the phone.
