Taking this survey will help the City of Minneapolis understand what food waste prevention resources are needed the most within the local restaurant community.
Only the City of Minneapolis Health Department staff will see your responses. Your answers will be anonymous, and no individual will be identified when analyzing the results. The information you provide will become public information and be accessible per Minnesota Government Data Practices Act, Minn. Stat. § 13. All unique identifiers will be removed.
Once you have submitted your responses, you will have the option to provide your contact information for a chance to win a roll of certified-compostable bags for your kitchen. Incentives will be mailed to winners.
