MIHEN Subaward Funding Request FY26

Prevention Network and the MIHEN are offering funding of up to $500 to 3 different colleges/universities/ organizations to prevent substance misuse on college campuses. Organizations that wish to utilize these funds must work with their community college/university on prevention efforts. Requests will be considered until funds are exhausted. 2. Must be located in Michigan (determined by physical address)
3. Must have a checking or savings account
4. Agree to attend at least 1 MIHEN meeting before October 1, 2026
5. Agree to have a 1:1 meeting with the MIHEN Program Coordinator before October 1, 2026

18.As an authorized authority for the above-named organization, you affirm that all information submitted is accurate and complete. Incomplete or contradictory submissions will not be considered. You should be contacted by the MIHEN Coordinator within two weeks of your submission. 

You understand that by completing this submission, Prevention Network is under no obligation to fund your proposal and any approved proposals will be subject to funds being available.

If approved, you agree to submit documentation of what the money was used for. All money not used must be returned. You will retain documentation of appropriate grant use, such as and including expense receipts, for the period of time that will be outlined in the award letter. 

You agree to submit a written summary report no more than thirty days after the completion of your proposed activities. You may also be invited to share at a MIHEN meeting or the MIHEN Conference. 

You covenant to save, defend, hold harmless and indemnify Prevention network, and all of its officers, programs, employees, and contractors from and against any and all claims, losses, damages, injuries, fines, penalties, costs (including court costs and attorney's fees, charges, liability or exposure, however caused, resulting from, arising out of, or in anu way connected with this application). You acknowledge that all award decisions are final and are not subject to appeal.

You acknowledge that false and inaccurate  response and statements made on the application are grounds for immediate rejection of the application.
