Dear Families,
You are receiving this benefits form because your student attends a Community Eligibility Program (CEP) school but does not qualify for meal assistance at home through programs like SNAP, TANF, FDPIR or, as a Migrant.
Completing and submitting this form is necessary for your school to secure funding through the Elementary and Secondary Education Act (ESEA) which may be used for after school activities, tutoring, technology, staff development, and parent classes.
To complete the form simply:
- Use the Income guidelines below to select one of the following: Eligibility 1, Eligibility 2, or Neither if neither chart applies.
- (Find the row with your family size. If your yearly or monthly income is less than the stated amount, you meet that eligibility level).
- Complete your parent/guardian name, email and phone number
- List your student's name, student ID number and school name
