Northeast District Department of Health Community Health Survey

The purpose of this survey is to get your opinions about community health concerns in northeast Connecticut. The Northeast District Department of Health (NDDH) promotes, protects, and improves health in twelve towns in northeast Connecticut through community partnerships. NDDH will use the results of this survey and other information to identify the most pressing matters which can be addressed through community action.
