About the Project:
Queensland Health has funded the Queensland Alliance for Mental Health (QAMH) to undertake the Mental Health Service System Changes: Experiences of COVID-19 Project. 

The Project is being delivered through a partnership between QAMH, Health Consumers Queensland, Metro South Addictions and Mental Health Services and Brisbane South PHN.

The aim of the Project is to understand the experiences of people who access mental health services, and those who support them, of the changes that have occurred across the mental health service system due to COVID-19.

We invite you to complete this survey to help us understand your viewpoint of service changes.

Privacy and Confidentiality:
All responses will be anonymous and confidential. All information you contribute will be analysed thematically with other responses and only prominent themes will be reported. In the case that a quote is reported, any identifying details will be altered to maintain confidentiality of participants.

You can skip any question that you do not wish to answer. The survey is voluntary and no remuneration will be provided. 

The survey will close 31 March 2021 and will take approximately 30 to 60 minutes to complete, depending on how much information you provide.
