As part of our ongoing effort to improve the Local Food Club and secure ongoing support, we are asking all Local Food Club participants to complete this survey.  Please complete this survey every month that you participate in the Local Food Club.

 Please complete all questions you feel comfortable answering.  When reporting on the findings, any identifying information will be removed and your responses will be anonymous.
