School Meals Experience Survey

The Lyon County School District (LCSD) school nutrition program is conducting a survey to learn more about the factors that impact the satisfaction of LCSD students with the school breakfast and lunch program.

Participation in this survey is completely voluntary. You may refuse to answer any of the questions on the survey at any time without penalty, prejudice, or loss of benefits.

This survey is anonymous and all information collected from this survey will be kept confidential. Survey results will be used by LCSD to address the needs of our students and provide the best possible child nutrition program.
