Hawaii State Council on Developmental Disabilities 5-Year Plan Public Survey
The mission of the Hawaii State Council on Developmental Disabilities is to create change that improves independence, productivity and inclusion for people with developmental disabilities and their families in community life.
To do this, the Developmental Disabilities Council identifies problems or needs that should be addressed. These problems or needs are agreed upon by Council members and identified in Developmental Disabilities Council's 5-Year State Plan on Developmental Disabilities. This State Plan is used to create specific grant proposals.
The 5-Year State Plan Public Survey will be the first step in identifying problems and needs. The Developmental Disabilities Council believes it is important to know what people like you find important.
In this survey, Developmental Disabilities Council means the full name of the Hawaii Developmental Disabilities Council.
We will not ask for your name or any other information that could be used to identify you. This information is being provided in a confidential manner, and will only be used to help the Developmental Disabilities Council develop their next 5-Year State Plan on Developmental Disabilities.
