Sign-Up Your Facility to be a Census Action Kiosk (CAK) Site

You can sign up your CAK Site(s) by clicking on this link (Excel file) to download and complete the survey. Please make sure to complete each column as it applies to the CAK you are registering, then save the completed survey to your computer and submit it via email to census@ceo.lacounty.gov.
