Louisiana Community Health Worker Survey

This survey is being conducted by the Louisiana Office of Public Health and the Louisiana Community Health Outreach Network (LACHON).  LACHON is a group that supports community health workers. 


Community health workers have lots of different job titles such as outreach worker, community health educator, community representative/coordinator/liaison, patient navigator, and patient representative.  What they have in common is that they are trusted members of the communities they serve.  Their job activities usually include doing outreach, education, advocacy, and/or case management to help people and communities improve their health.
