People of color and underserved populations remain underrepresented in colorectal cancer (CRC) screening, care and treatment. As a primary care clinician, your recommendation and referral of patients to CRC screenings, care and clinical trials is an important step to promoting equity.
 
This practice assessment survey can help point to education and quality improvement strategies that will best help you safely and effectively refer your patients to get screened, as well as help eliminate disparities in CRC.
 
The first part of this survey is focused on general issues related to your practice; section two focuses on colorectal cancer and your patients within the practice.
 
One person in your practice knowledgeable about CRC screening activities that you implement should complete the survey. There is no need to look up data in your EHR, just estimate answers to questions about the patients in your practice.
