20,000 Homes Campaign Support Options Sign-Up for 2018-2020

We are excited to offer participating 20,000 Homes Campaign communities one of three support options beginning in November 2018 to accelerate efforts towards our collective goal to end chronic homelessness in 20 communities by July 1, 2020.

Communities are asked to sign-up for one of the three support options outlined in this 20KHomes Campaign Support Options 2018-2020 Description document by the Sign-Up Deadline - September 5, 2018.

Campaign Leads from each community should be the ones to sign-up their community. If you are not the Campaign Lead, please contact them to discuss this opportunity further.

4. Organize an “away team” to attend the Learning Sessions and a “home team” that will support implementation during the Action Cycles.

5. Submit data monthly (see original sign-up commitments for further information).

6. Submit Scorecards quarterly (until completed).

7. Share your data (as outlined in the Reduce Description and Q&A).

8. Work towards a quality By-Name List and Coordinated Access System as quickly as possible.

9. Participate in Learning Sessions (identifying goals/improvement projects), Action Cycle testing, coaching, webinars, and other supports that may be available to you.

10. Share your ideas, projects, bright spots, tools, and lessons learned with other Campaign communities.

11. Have already submitted or will submit at least one By-Name List Scorecard, one Coordinated Access System Scorecard and at least one month of move-in data by October 19, 2018 in order to participate.

12. Pay participation costs to CAEH (as outlined in the footnotes of the Support Options Description) by no later than May 31, 2019.
