Question Title

* 1. As of May 13, Oregon has a multi-phase approach to re-opening. It is appears possible, perhaps likely, that Oregon will be in a "Lower Level" of risk this fall. This level appears to allow as many as 300 attendees at an indoor event (though guidance is not entirely clear) with proper distancing and protocols.

Generally speaking, how willing are you to participate in an indoor conference with up to 300 attendees if that event were held today?

Question Title

* 2. Have you been fully vaccinated yet, or do you intend to become fully vaccinated by July 1st?

Question Title

* 3. We plan to hold our 2021 Oregon State of Reform Health Policy Conference on October 26th. By October 26th, do you believe you will feel comfortable attending a conference with up to 300 attendees in downtown Portland with proper distancing and protocols?

Question Title

* 4. If we limited the total number of in-person attendees to 100, would you be more or less likely to join this event in-person, given the trade-off between safety and networking opportunies?

Question Title

* 5. We are considering offering a hybrid option for attendees, where one could choose to either be in person or join us virtually. We could provide the flexbility to change the type of participation up to 14 days before the conference, but it would mean higher rates for everyone of about 5-10%.

Knowing this, would a hybrid offering that was slightly more costly for everyone ($299-$349) make you more or less likely to register to join us via either option (in-person or virtually)?
