To assist you with the Disciple Maker Index process, we are asking that you appoint a Parish Point of Contact (PPOC). This survey will collect the contact information for your PPOC. Please fill out all fields below.
The Parish Point of Contact will help you manage and communicate the survey to your parishioners and will receive a log-in for the Catholic Leadership Institute portal, where they will have access to administrative functions for the Disciple Maker Index.
