This survey is about health and social behavior. The information you give will be used to develop better health and social services for young people like yourself. Your answers are anonymous.
There are a number of different screens with 4-10 questions on them about a specific topic. The whole survey should take you about 20 minutes to complete.
Once you have finished the survey you will be given the opportunity to enter into a lottery where ten $15.00 gift certificates will be given away. The instructions on how to enter the lottery for the gift certificate drawing is located at the end of the survey.
