This survey is designed to gather feedback from the community about out-of-school time programming for children and youth (i.e. afterschool and summer activities). It is designed to take 5-10 minutes. The Community Development Division (CDD) of the City of Madison will use your feedback to inform our upcoming funding process for child and youth programming.

If you want to share feedback on programs for multiple age groups, for example, for Elementary school age programming and Middle school age programming, please consider taking the survey twice so we can capture your feedback for each age group.
